Occasionally, individuals will make mistakes when completing a form. They may misunderstand the directions or evaluate the wrong person or program. In these cases, they may ask you to reopen the form so they can make corrections.

Be cautious when asked to reopen a form by a resident who has already seen the results of their own evaluations. You must guard against individuals making changes in response to an unsatisfactory personal evaluation. |
To reopen a form:
1 Perform a Custom Form Search (if you don't know how, please see To Perform a Custom Form Search).
2 In the new window, scroll down until you see the form you want to reopen. Click "reopen."
