Each form that is attached to a rotation represents a specific evaluation process. For example, in the image below, the rotation called "BC Womens Fetal/Mat Med" has three forms, and therefore three evaluation processes, attached to it.
When you remove a form from a rotation you are actually cancelling an evaluation process. Once a form is removed it will no longer be included in the block sendouts for that rotation.
This cannot be undone. If you delete an evaluation process from a rotation by mistake, you will have to re-attach the form. For instructions on how to do this, please see Attaching Forms. |

To remove a form from a rotation:
1 Click the Setup tab.
2 Click the Evaluation Workflow link. A list of your rotations and the forms you've attached to them appears.

3 Scroll down until you see the form you want to remove. To the right of the form, click "delete."

| You are not deleting the form itself. You are just canceling an evaluation process from one specific rotation. The next time you perform a block sendout, the form will not be included for that rotation. In addition, when you remove a form you will not delete any data that has already been collected. Evaluations that have previously been sent out (whether completed or not) will not be affected. |
